Patient Care Co-ordination Centre
The NHS Trafford CCG has commissioned the new PCCC to offer joined-up solution to deliver seamless, coordinated, quality care for all patients and service users in Trafford, North England. The new centre will provide a single point of access and monitoring of care to ensure that all patients receive “the right care at the right time at the right place.”
To deliver and co-manage the new centre in Trafford, the NHS Trafford CCG selected IT services firm CSC. Under the contract, the consulting firm will provide a complete end-to-end offering, including initial patient communications, clinical workflow based on a catalogue of services, and back-end analysis of the effectiveness and efficiency of care delivery.
Commenting on the awarded contract, Nigel Guest, Chief Clinical Officer at Trafford CCG, says: “The PCCC is a key element of our strategy for delivering high quality, co-ordinated healthcare to the Trafford population. In CSC we are delighted to have found a partner with a strong track record of delivering innovative solutions in complex and challenging environments including the NHS.”
Philippe Houssiau, Vice President of UK Healthcare at CSC, adds: “We are delighted to be working with the PCCC team to deliver what will be a new standard of care coordination for the Trafford population. The PCCC is designed to improve the coordination of care across primary, acute, community and mental health sectors and deliver a significantly improved experience for patients and care providers. Together we will be delivering a high quality, value for money service, which should lead the way for how coordinated care is delivered across England.”
The new PCCC will be based in Altrincham and is expected to be in use by the summer of 2015.
